Safety First: For your child's safety, only persons designated as authorized on the registration form will be able to pick up your child from program. The Authorization Card and Picture ID MUST be shown when picking up your child.

Absents: Please call the Kalihi YMCA at 848-2494 by 8:00 am everyday your child will be absent from the program. There is no refund for days missed.

Be Healthy: The Kalihi YMCA recognizes that nutrition is important for children. Therefore lunch and snack times are part of the daily schedule. We recommend that children bring enough food and drinks for themselves. Sharing of food is prohibited among participants because of various food allergies and dietary restrictions. The Kalihi YMCA also encourages children to eat before coming to program.

Sun Safety: Although we try to avoid scheduling activities in the sun for extended periods of time, children are expected to participate in outdoor activities. We highly encourage children to bring water bottles and drink lots of fluids. We also support the use of sunscreen, sun shirts, and hats. Be 'sun smart' and teach your child how to be responsible in the sun.

Cancellations/Program changes: Please call 848-2494 as soon as possible if you need to change your child's program. Cancellations must be in writing and received at least 7 days prior to the start of the program. $50 of your payment is non-refundable. The Kalihi YMCA reserves the right to make program chages due to enrollment

Scholarships: The Kalihi YMCA believes its programs and services should be accessible to everyone. Scholarship applications are available.

Emergency Care Agreement
I hereby agree, that in the judgment of the YMCA supervisor on duty, my child requires medical care; and the YMCA staff is unable to contact me, those named as emergency contact persons, or the child's physician, the YMCA is authorized to take my child to the nearest medical facility for treatment deemed necessary by the personnel of the medical facility.

Video/Photograph Release
I hereby authorize the YMCA to use my child's name and photographs or video at any time in any manner in connection with its advertising, publicity, and/or public relations programs. The video-photo may only by used by the YMCA. I will make no further claims.

Late Registration: There is a $40 late fee if registering after Friday, June 2nd, 2006
